Rehabilitation Case Management

Rehabilitation Case Management is a process which assesses the individuals’ needs following injury, provides recommendations for health care, home adaptations, housing needs and equipment to assist with disability, benefit advice and assistance to return to work.

A comprehensive written report is presented to the referrers in order to obtain authority to implement the recommendations. The case manager then co-ordinates, monitors, and evaluates the recommendations ensuring they meet an individual’s health needs.

The benefit of Case Management can be measured by earlier return to pre-injury lifestyle and employment.

The person centred approach of case management provides ongoing support during the journey of recovery.

Ongoing Case Management

Ongoing case management to implement recommendations made within the initial needs assessment. The aim is to provide cost effective treatment in order to assist individuals, who have suffered a personal injury back to work and a pre-injury lifestyle.

The rehabilitation code

We work within the spirit of the rehabilitation code and the INA report includes the 10 markers highlighted in the 2015 rehabilitation code.

Initial Needs Assessment

The Case manager will carry out a face to face assessment of the injured person. This may include a telephone interview prior to appointment.

The following points will be included: -

  • The physical and psychological injuries sustained by the injured person and the subsequent care received or planned.
  • The symptoms, disability/incapacity arising from those injuries.
  • The availability or planned delivery of interventions or treatment via the NHS their employer or health insurance schemes.
  • Any impact upon the claimants domestic and social circumstances including mobility, accommodation and employment.
  • The type of clinical intervention or treatment required in both short and medium term and the reasoning for the intervention.
  • The likely costs and duration of recommended interventions or treatment.
  • The anticipated clinical and return to work outcome of such intervention or treatment.

Teresa has developed skills to deal with catastrophic cases since 2003 having an excellent success rate in returning individuals back to work. She also has supported individuals to identify new career options, liaised with employers, devising phased return to work plans and supporting the employer and the individual to sustain the work role.

She has a caring approach to proficiently deal with rehabilitation clients, with good communication skills liaising with stakeholders involved in rehabilitation clients. She achieved Case Manager of the year 2015, is committed to the development of Case Management with the UK and is currently Vice Chair for CMSUK.

Lesley Macdonald

Lesley has had a career within the field of case management since 2003 and has achieved success with clients supporting them through rehabilitation and return to work programs. She has managed complex and serious cases including multi trauma, complex orthopaedic, amputations, brain trauma and psychological problems.

As a senior clinician, she establishes an excellent relationship with all grades of staff including doctors, social workers and lay personnel. She also liaises with the community care management teams, ward based teams, health authorities and primary care trusts, in various parts of the country.
